Open AccessJC: July 2023

This study evaluated the relationship between excision margin size and local recurrence risk in T1a melanoma located near critical anatomic structures. The findings supported that narrower margins may be acceptable for thin melanomas in cosmetically or functionally sensitive areas without significantly increasing recurrence risk.

Take-Home Messages

  • Narrower excision margins for T1a melanoma near critical structures do not appear to significantly increase local recurrence rates.
  • Margin decisions for thin melanomas at sensitive anatomic sites should balance oncologic safety with functional and cosmetic preservation.

Abstract

Melanoma guidelines recommend surgical excision with 10-mm margins for T1 melanoma. However, this procedure may be problematic at sites close to critical structures such as the scalp, face, external genitalia, acral, periumbilical, and perineal areas. To compare outcomes of wide (10-mm margins) vs narrow (5-mm margins) excision in patients with T1a melanoma near critical structures. This cohort study was a retrospective comparison of 1341 consecutive patients aged 18 years or older from the N...
